The digital identification form is constructed based on the templates provided by OVAM.
This digital identification form is unique for Belgium and meets the requirements of the 3 regions.
The PDF document provided is bilingual: French and Dutch (titles and labels), but the codings made by users are in the original language.
The digital identification form is signed and dated by the IHM or AZRT prior to the transport activity.
The pdf document contains the most recent information entered into the system. If the information differs from the information at the time the identification form was issued, a clear note will request that the change logs be consulted for further details.
Each change or addition shall be recorded and frozen with the name of the person who made the change or addition and the date and time it occurred.
This applies particularly to the WEIGHT of the waste to be transported when the quantity cannot be determined prior to departure or is only estimated and must be determined or corrected upon arrival at the destination.
OVAM imposes a data exchange format and a set of mandatory fields:
For non-hazardous waste
(1) unique sequence number beginning with “UBI” (chosen format is UBI-XXX-XXX-XXX-WFL) that identifies the system used to create the identification form;
(2) date of transport as provided by the COLLECTOR;
(3) name and address of the producer of the waste and the delivery address of the waste specified by the transporter;
(4) name, address and identification number of the COLLECTOR;
(5) name, address, and identification number of the carrier;
(6) name, address and identification number of the processor.
(7) description, quantity in tons and the EURAL codes of the waste and R&D codes + processing technique (see https://ovam.vlaanderen.be/r-d-codes)
Weights do not need to be entered at the start of transport, but should be entered before the document is completed to ensure it meets legal requirements.
Description of waste: the system allows the user to change the EURAL descriptor to their own description.
(8) geolocation of the start of the TRANSPORT activity, as X and Y coordinates in Lambert 2008 coordinates. The driver must validate the start of the transport on his mobile device. The X,Y coordinates are collected when pressing the START TRANSPORT and SIGN for DELIVERY buttons.

For hazardous waste
In addition to the information required for non-hazardous goods, the user must provide the following information
– Physical properties of the waste;
– Chemical composition of the waste;
– type of packaging;
– number of packages;
– Special instructions for transportation, if applicable.
We provide as many additional information fields as there are hazardous wastes included in the shipment.
ChangeLog
All changes or additions to the document after it is signed by the collector are recorded and frozen with the name of the person who made them and the date and time they were made.
since a person is tied to a company in our system, we can add the company information if desired.
This is especially true for the WEIGHT of the waste to be transported, when the quantity cannot be determined before departure or is only estimated and must be determined or corrected upon arrival at the destination.
The exact weight is delivered to all actors after identification form has been given ‘FINAL’ status by a service sending an email with attached pdf files (identification form + change logs) to all actors involved in the shipment.
This attachment must be kept with the original identification form.
The log of all changes, such as date, source, change/addition associated with a form, is available to inspection services from the time of inspection and if required by supervisors and other authorized inspection services along with the form data or visualization of the form.
Interoperability
Within the framework of interoperability between the different systems, mandatory fields have been defined by the OVAM.
